Chronic Jaw Pain, Grinding, and Tension

If you wake up with a sore jaw, notice tension headaches by mid-afternoon, or catch yourself clenching during the day, you’re not alone. Jaw pain, teeth grinding (bruxism), and tension in the muscles around the temporomandibular joint (TMJ) are common — and they’re usually treatable.

We take a straightforward approach: figure out what’s driving the pain, then choose the least-invasive treatment that’s likely to help.

Common Signs of a TMJ Problem

  • Jaw soreness — especially in the morning
  • Clicking or popping when you open your mouth
  • Tension headaches, particularly at the temples
  • Neck and upper-back tightness
  • Worn, flat, or chipped teeth
  • Difficulty opening your mouth fully
  • Ear pain or a feeling of fullness in the ears without an infection

Not all of these mean you have a TMJ problem — but if you’re noticing several, it’s worth a conversation.

How We Treat It

Custom nightguards

For patients who grind or clench at night, a properly fitted nightguard protects your teeth from wear and often reduces morning soreness. Store-bought guards can help in the short term, but a custom nightguard is more comfortable and lasts longer.

Therapeutic Botox

For chronic tension in the muscles that close the jaw (masseter and temporalis), therapeutic Botox can reduce muscle overactivity and relieve associated pain. This is a therapeutic use — not cosmetic — and results typically last 3–4 months per treatment.

Bite adjustment and restorative work

If uneven pressure across your teeth is contributing to the problem, a small adjustment to the bite or restorative work to rebuild worn teeth may be part of a longer-term plan.

What to Expect at Your First Visit

We’ll ask about your symptoms, look at how your jaw moves, check the muscles for tenderness, and examine your teeth for wear. From there, Dr. Yates will walk you through a plan — which might be as simple as a nightguard, or might include Botox and follow-up appointments.

You’ll leave with clear next steps and a written estimate before any treatment begins.

Does therapeutic Botox for jaw pain hurt?

Most patients describe the injections as a pinch. The whole treatment usually takes just a few minutes, and there’s no downtime afterward.

How long does Botox last for jaw pain?

Results typically last 3–4 months. Some patients find that after a few rounds, they can space treatments further apart.

Is this covered by dental insurance?

Coverage varies. Nightguards are commonly covered by dental plans. Therapeutic Botox is often not covered by dental insurance, though some medical plans cover it when it’s treating chronic pain. We’ll help you understand your options before treatment.

Will a nightguard alone fix my TMJ pain?

For many patients, yes — especially when nighttime grinding is the main driver. For others, a nightguard is one part of a broader plan.

How do I know if I’m grinding at night?

Common clues: morning jaw soreness, tension headaches you wake up with, or a partner who’s heard you grinding. Wear patterns on your teeth are another giveaway, and Dr. Yates will look for those at your exam.
